Documenting a Concern So It Can Be Acted On

A template approach for turning an experience into a complaint or report that an institution can actually investigate.

Facts before conclusions

Describe what happened, when, where, and who was present, before describing what you believe it means. Investigators act on facts.

One concern per submission

Bundling several issues into one complaint often causes all of them to be handled slowly. Separate submissions are usually clearer.

State the outcome you are asking for

Say plainly what you want to happen. A complaint without a requested remedy invites a response that resolves nothing.
